STAT Home Health is for patients who need traditional healthcare services in their homes, such as skilled nursing or therapy.

Part of our at-home healthcare services, palliative home health services allow patients to receive the palliative care they need while also maintaining the important independence and comfort associated with their own homes. AIM Palliative Home Health helps support patients who are undergoing aggressive forms of treatment, even during late stages of an illness, and patients who are not ready to transition into hospice care. Homedica’s Palliative Medical Calls division can also provide palliative care to patients with similar needs.

Our house call doctors and nurse practitioners are available to provide primary home care when patients need it. Our patients can receive the skilled medical care they need during times when getting to their physician’s office is made difficult by limited mobility or a depressed immune system. Homedica’s Medical House Call and Palliative Medical Call teams provide primary care to patients in the comfort and security of their own homes, making quality healthcare easily accessible no matter what a patient’s circumstance. This team helps fill the gap in care until patients can get back to their physician’s office for visits.

We understand that unexpected times of need can often be the most stressful. That is precisely why we offer continuous, in-home crisis care. Our Hospice Crisis Care team provides at-home, intensive hospice support around the clock at bedside. We’ll provide emergency support for up to 24 hours (with patient appropriateness reassessed every 24 hours, according to CMS guidelines), ensuring we address patients’ needs in a timely, thorough manner.

Transitioning out of the home can be a difficult step to take; but at Capitol House Nursing & Rehabilitation Center, a welcoming community makes the change as smooth as possible. Capitol House is a skilled nursing and rehabilitation center that residents are proud to call home. The team at Capitol House provides general support as well as specialized care for changing needs over time.

We recognize that terminal illness spans beyond physical health, therefore we offer a holistic hospice approach that addresses the unique physical, emotional, spiritual, and social needs of each patient. Terminally ill patients can seek hospice services from St. Joseph Hospice, or find care within The Carpenter House. Putting peace, comfort, and dignity first, staff members support patients and families as they face the inevitable challenges ahead.

Grieving is a natural reaction to the loss of a loved one, and each person has a different healing process. At St. Joseph Hospice, friends and family can find hope and peace within support groups, programs, and resources for a full year following the loss of a loved one.

Rehabilitation can empower patients to remain active and help them embrace each day with more energy and confidence. Sage Rehabilitation Hospital & Outpatient Services provides both inpatient and outpatient rehabilitation services to make this possible. From short-term to long-term support, Sage offers a variety of customized rehabilitation programs to help patients gain strength and mobility. For patients who need rehabilitation services as well as nursing care, Sage also offers service within Capitol House.

Often, it’s the everyday tasks that can make patients feel the most accomplished, but sometimes patients need a helping hand or word of encouragement to achieve them. Companion Home Services assists individuals with day-to-day needs and provides companionship to help keep seniors active and engaged. This team can help individuals live a longer, more fulfilled life as they recover from or cope with illness, injury, and/or aging.

Independence is one thing that illness or injury should never take away — and private duty care ensures that patients retain their sense of independence no matter what their circumstance. At The Carpenter Health Network, we provide private duty care to make sure that patients receive the assistance they need in the comfort of their own homes. From nursing to rehabilitation, we provide a number of private duty services.

St. Joseph Hospice volunteers share the desire to enrich the lives of others by giving their time. Our dedicated volunteer group understands the challenges that patients and their families face when coping with injury, surgery, or illness. In response to these hardships, our volunteers offer support to help patients treat and manage their symptoms with hope, courage, and the support of a community.
